What's the Order of Initials in a 3-Letter Monogram?

When representing a single person, usually 3 letter monograms are in the following order:

First Name Initial
Last Name Initial
Middle Name or Maiden Name Initial

with the Last Name Initial in a larger font size than that two surrounding initials.

So for example, if your name is Jennifer Mary Knowles, traditionally your circle monogram would be: JKM
